The Community Support Register gives you confidence that if you are unable to provide information (say, due to accident or illness) your medical details and who to notify are readily available to the relevant authorities. The Ballarat & District Community Support Register is a project of the Ballarat Neighbourhood Centre.

Activities

Details are entered and managed on a confidential computer database. Authorised volunteers will provide telephone welfare checks to participants on a regular basis. They may also pass information onto emergency services if needed.

This is a free service.

Linkages provide Case Management and Brokerage services to support an individual's choice to remain at home. The services provided will promote the person's independence and dignity and will contribute to their physical, emotional and social wellbeing. Case management is a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual's health needs through communication and available resources to promote quality cost-effective outcomes.
